Lab Results | 21st Century Cures Act

The 21st Century Cures Act provides immediate access to laboratory results to patients. This means your lab and test results, including bloodwork, STI tests, and pathology reports, will be visible as the tests are complete and automatically post in our computer systems.

Unfortunately, this means you will likely see results before we can review them with you.
Depending on when your results are delivered, your VWC physician may be busy caring for other patients, at the hospital in surgery or labor and delivery, or out of the office. A physician or a member of their team will contact you regarding your results and make necessary care recommendations as soon as they are able.

FMLA/Disability

FMLA/Disability

FMLA/Disability paperwork is complex and takes time away from our ability to care for you when you’re in the office or during phone consultations. We recognize that accurate and timely completion of these complicated forms is critical to you receiving your FMLA benefits.

Please ensure your name is on all paperwork—the FMLA/Disability Authorization form(s) AND your company/organization’s paperwork—especially when submitting paperwork on behalf of your partner or family member. 

1) To initiate your FMLA/Disability request(s), complete either the:

2) Deliver the completed authorization/release and the blank FMLA and/or Disability form(s) to be completed to your VWC provider’s office, or you may fax them to 804.282.8678.

3) A fee of $30 per FMLA & Disability submission is required to initiate the data collection process and complete the necessary paperwork. You may pay the required fee(s) in the office when you drop off your paperwork, online through your myPrivia patient portal, or by phone 804.282.1095.

4) Once payment is made in full and forms received, your paperwork will be completed within five (5) business days and sent to the recipient designated on your FMLA/Disability Authorization Form(s).

To find out if a particular physician or clinician participates in your plan, call 888.774.8428. Some insurance plans require you to obtain authorization before seeing us. We encourage you to determine the extent of your benefits before initiating treatment.

Most insurance companies cover annual well-woman exams. Most insurance plans require a co-payment or a payment toward your deductible at the time of your visit for all other services. To make it easy, we accept checks, VISA, Mastercard, Discover, FSA and HSA cards.

Please bring your insurance card, valid photo ID, and co-payment or payment toward your deductible to each visit.

If your plan requires preauthorization or precertification, we’ll help you obtain benefits and pre-certify your services so that you can receive the maximum benefits for your medical care.
